Confirm Coverage

Contact your insurer to confirm that your treatment at Midland Eye is covered under your policy, as coverage may vary across different insurance providers.

 

Obtain Authorisation

Before booking any consultations, diagnostic imaging, or surgery, ensure you have received prior authorisation from your insurer to cover the treatment.

Receive Authorisation Details

Once authorised, your insurer will provide you with an authorisation number or claim form. Make sure to have this ready before your appointment.

Bring Authorisation and Membership Details

When you come in for your appointment, bring the authorisation number or claim form, as well as your Membership, Policy, or Plan number for verification.

Direct Billing or Payment

We can arrange direct billing with your insurer for most treatments. If direct billing is not possible, you’ll need to pay upfront and seek reimbursement from your insurer.

Speak to your GP or Optometrist

For most hospital services you will need to be referred by your GP or Optometrist before you can be seen at any UK hospital. When your GP or Optometrist recommends that you need to see a specialist consultant, ask them to refer you to Midland Eye.

Book Your Consultation

Once you have a referral from your GP, you can then book an initial consultation directly with us at a time to suit you – this would cost between £100 and £230 depending on the type of consultation.

If you have a specific consultant in mind we can put you in contact with their secretary for their individual charges. If any tests are carried out at the initial consultation they will be charged in addition to the consultation fee.

Arrange a Treatment

If once you’ve had your initial outpatient appointment, your consultant recommends that you need some further tests or surgical treatment we will let you know exactly how much your initial treatment will cost before going ahead and then arrange a time convenient for you.

Pay for Treatment

Wherever possible we offer all-inclusive package prices to include treatment and a post-operative appointment. Costs not included are costs associated with unrelated medical conditions, additional treatment and further tests.

Common questions

Here are some frequently asked questions from ophthalmic patients just like you.

You will need to complete a simple, online application, which one of our team can help you with. The information is transmitted to the lender immediately and a reply is usually received by return. You can then either sign a credit agreement.

You can borrow between £1,000 and £25,000. You must be over 18to apply, and you must have lived in the UK for at least 3 years.

 

A deposit is not required, but if you want to pay one and so reduce the amount you borrow, this is completely up to you.

 

This will depend on the amount you borrow and whether you pay a deposit. Our staff will show you, completely without obligation, the monthly repayments for your particular treatment.

The monthly payments are collected by Direct Debit.

Immediately – as soon as the credit agreement has been signed.

There is a statutory ‘cooling off’ period of 14 days after signature during which you can cancel the credit agreement with no cost to you. However, if you have started any treatment which would have been financed, you will have to make other arrangements to pay for it.

The finance is arranged through Chrysalis Finance Ltd., a company authorised and regulated by the Financial Conduct Authority to carry out the regulated activity of credit broking under Firm Reference Number 631193
